Enrol at the Health Centre
Being enrolled at the University of Canterbury does not automatically enrol you at the health centre - even though you have paid your health centre levy.
You are very welcome to use the health centre even if you have not enrolled as one of our patients, but it will cost you less if you do enrol with us.
- Print off the form Patient Enrolment Form (PDF, 123KB)
- Fill in the form
- Please mail or hand in at the Health Centre your completed form. (The Ministry of Health requires that we may not accept electronic signatures so we cannot accept emailed forms).
