Description

This course will introduce students to health service contexts and the complexity of care across a range of settings. This is achieved through comprehensive health assessment and the design of care plans leading to considered interventions underpinned by evidence based practice.

Indicative Course Outline

- Body systems approach to major acute and long term physical illnesses – aetiology, pathophysiology, treatment and care management
- Management of disease including responses to surgical and medical interventions, sick child, infection control, wound, fluid and electrolytes and pain management
- Experience of Illness including pre and post-operative care, co-morbidities, living with long term conditions, rehabilitation and community resources
- Government health priorities and care rationalisation – the impact on individuals and families / whanau
- Introduction to Palliative Care
- Evidence based care workshop
- Pharmacodynamics, pharmacokinetics, therapeutic drug monitoring
- Medications used in treatment of major physical conditions, adverse drug reactions and lifespan considerations

Learning Outcomes

1. Synthesize, integrate and apply the epidemiology, pathophysiology and care management of major physical health changes and the impact these have on individuals and whānau/family in relation to acute, chronic, rehabilitative and palliative phases.
2. Critically evaluate and discuss issues associated with chronicity and co-morbidity as it relates to the healthcare and illness experiences of individuals and whānau/family.
3. Critically evaluate the responsibilities of providing competent nursing care when practicing within the inter-professional health care environment.
4. Explain the mechanism of action of selected drug categories and critically discuss care activities which maximise therapeutic effectiveness and minimise adverse reactions.
5. Utilise case studies to Integrate knowledge and explain drug interactions, adverse drug reactions and contraindications including complementary medicines.
6. Utilise evidence based practice to support best quality health care provision and outcomes in acute and long term health care settings.

Assessment Information:
Assessment 1 – The Patient’s Journey: Management of one pathophysiological condition.
Due Date: Wednesday 8th August 17:00
• Word Limit: 3500 words (excluding references)

Instructions:
You will be assigned one pathophysiological condition from the generated list. Using the following bullet points as your headings, write an assignment to cover the key aspects of your allocated condition. Individualised sheets for each condition will be developed and allocated to a student at the first study block. This will itemise what is to be covered under each of the main headings in the following template.
• Epidemiology
• Pathophysiology
• Clinical presentation
• Assessment- Identify potential assessment findings including diagnostic tests and investigations. You must include findings from nursing assessments here.
• Management of care
• Medical: Interventions including pharmacotherapy- using framework provided in HLTH466
• Interdisciplinary: team contribution as relevant
• Complications
• The lived experiment - You will search for and share some research related to the lived experience of people with this condition.
• Track the patient's journey from acute phase to home, including community agencies involvement, rehabilitation, long term.
You are required to:
• Use the APA 6th Edition referencing
• Submit your work, as a WORD document, to ‘Turn-it-in’ via the HLTH468 UCLearn Site on or before the due date.
Each student will be assigned one pathophysiological condition to research and present. In addition, an academic staff member will present Hepatitis and Liver Failure using the template as a teaching example. In addition, a range of other topics will be covered in class or on LEARN, these will include but are not limited to tuberculosis, traumatic brain injury, spinal injury, and Parkinson's Disease.

Assessment 2 – The Patient’s Journey: Presentation

• Powerpoint Presentation (40 minutes, plus 10 minutes for questions)

• Instructions:

This assessment is designed so you can share with colleagues your in-depth understanding of one pathophysiological condition and the effect this illness has upon an individual and their significant others.
You are required to prepare a PowerPoint presentation with notes that summarise the key aspects of your condition from assignment one, using the same headings from the template. In addition,
• You will search for and share some research related to the lived experience of people with this condition
• Track the patient’s journey from the acute phase to home including discharge planning, community agencies involvement, rehabilitation resources, long term outlook, palliative care or anything else you think relevant for allocated condition. If possible it is anticipated that you will arrange to meet with local agencies/ providers and share useful resources in the class.
After the presentation, you are required to make any changes to the presentation in response to feedback and email to academic staff for loading on the HLTH468 UCLearn site.
You are required to:
• Bring your Powerpoint presentation on a USB stick to class on the due date.
• Use the APA 6th Edition referencing
• Submit your PowerPoint to academic staff by Friday 8th September

• Assessment 3 – The Patient’s Journey:  Examination. 3 hours
• Due Date:  Wednesday 4th October, time and location to be advised
The examination will consist of multiple choice question s(MCQ) and short answer questions (SAQ) which will draw from material from textbooks related to the pathophysiological conditions, pharmacology, the students presentations and other content covered in the two blocks.
